 H4 extension address use. Do I need to use her address or mine US address is fine?

Which address should I use for dependant for H4 extension. She just moved to Canada due to uncertainity of green card in  US, it is just taking long time. As we are in last 2 years of Canadian PR we decided it is better if she moves there for time being.

So do I use her Canadian address for her h4 extension or can I use my USA address for her? The thing is she may not be staying at her current address for long. By the time, immigration approves and sends it, she will have moved.

Also, is there a mutltiple entry h4 visa?? Otherwise whether it will be OK for her to apply for the tourist visa?
