Make an infopass appoinment using uscis.gov site. Fill I-102 form and give explanation to officer on what had happened to the I-94 form. They will then issue a new corrected I-94.

USCIS corrects only information entered by them is mistake. If you did mistake in filling out form -say misspelled name or date of birth, then you have to go to Custom Office (Mostly port of Entry) and make correcrtion. USCIS does not handle such cases.

Yes nepal_ko_lato, is right it is better to go to port of entry and correct it. I had same problem. It takes only about 15 minutes..

i had the same problem n still in trouble. my date of birth was wrongly entered in the Port of entry in San Fransisco. i had no idea about the consequences....my destination was Colorado. i came here. in my university, on campus job is quite difficult to get...after about 6/7 months i got a job .Then the problem started. i went to apply for my SSN but then they gave me a refusal letter saying my date of birth in the INS office record does not match with that of my passport!!!! i was really worried because i had no clue where to go. some said, i had to go back to San Franscisco some said somewhere else. i talked with my international advisor n she said she'll try to correct through her...after a month i went to her n asked wats happening....she said she was very bg n could not do....then i decided to do my work myself!!!! well i was confused where to go....then i heard, to make the corrections i COULD GO TO THE NEAREST INTERNATIONAL AIRPORT...WELL I WENT IN THE dENVER INTERNATIONAL AIRPORT AS IT WAS NEAREST 4 ME...IN AN INTERNATIONAL AIR PORT U'LL C THE IMMIGRATION OFFICE...U CAN GO THERE N MAKE THE CORRECTION....THE WAY I DID.....u need to do this as soon as u can coz it keeps ur lots of work pending.....
